FIREARMS LICENSING (JURIDICAL)
Requirements for New/Transfer Firearms License
(Juridical)
- An applicant who is an
employee of said juridical entity must be Filipino Citizen, at least 21
years
of age, of good moral character and qualified to
posses/own a firearm. Application form must be properly accomplished, signed
by the applicant and duly notarized. The following forms are to be
strictly complied with before submission:
- PNP Form No. 2 (Application
for Regular License)
- Information Sheet
- Duly accomplishes Firearm
License Application (3 copies duly notarized w/ CTC No)
- Affidavit of No-Pending Case
- PNP Form No.12 (Permit to
Purchase Firearm and Ammunition)
- Firearm License Fee (EO256)
and Surety Bond
- Deed of Sale or Affidavit of
Transfer (For Transfer Firearm)
- Long Envelop for application
For Private Agency/Company Security Force,
only a and c above will be accomplished prior
to submission of the application to SOSIA for further
compliance of SOSIA requirements and for its subsequent endorsement and
processing.

Proof of citizenship for foreign sounding name
(Birth Certificate, proof of naturalization
issued by the local registrar, Passport, Voters ID)

(nature of work/business or Certificate of Employment)

2x2 and two (2) pcs 1x1 ID pictures with signature
- Police Clearance (Place of
residence)
- Court Clearance (Place of
residence)
- Mayor's Clearance (Place of
residence)
- Income Tax Return duly
notarized by BIR representative
FOR PRIVATE FIRM / ESTABLISHMENT /
CORPORATION (Long Regular License -LRL)
- Comply requirements 1a, 1c,
1e and 1h above (Accomplished in the name of the said entity)
- Balance Sheet and Profit and
Loss Statement showing the capital investment and gross sale
for a quarter period (Duly certified by a CPA or the
City/Municipal Treasurer)
- Certificate of
Registration with the Securities and Exchange Commission (SEC)
FOR PRIVATE FIRM WITH LICENSED FIREARM FOR
USE OF ITS EMPLOYEE (SRL)
- Comply requirements 1a to 1h
above
- Three (3) pcs 2x2 and two (2)
pcs 1x1 ID pictures with signature
- Certification
from the owner or manger that applicant (excluding security guard) is the
user
of the firearm applied for (Such firearm
already covered by LRL)
FOR GOVERNMENT AGENCY / OWNED OR
CONTROLLED CORPORATION FOR (LCR)
- Application must be in the
name of the Government Agency or Controlled Corporation
- Accomplished
PNP Form No. 3 (Signed by the Department Secretary or Bureau Director or
Head of Office with equivalent rank)
- Approved requisition and
Issued Voucher (RIV) & Council/Board Resolution
- Deed of Sale or Affidavit or
Transfer (For firearm acquired from a licensed holder)
FOR GOVERNMENT AGENCY WITH LICENSED FIREARM FOR USE OF ITS
EMPLOYEE (SCR)
- Comply requirements 1a to 1h
above
- Three (3) pcs 2x2 and two (2)
pcs 1x1 ID pictures with signature
- Clearance from PNP
Directorate of Intelligence (DI) - formerly C-2
- Appointment Order from the
Civil Service Commission (CSC) or Oath of Office in case
of Elective Official
FOR PRIVATE SECURITY AGENCY
- PNP Form No. 12
- Application for Firearm
License (3 copies duly notarized)
- License to Operate (LTO)
- List of Guards and Number of
Firearms
- Board or Council Resolution
for the purchase of firearms (Resolved by LGU Officials)
- Endorsement from SOSIA, CSG

Requirements for Renewal of Firearms License
(Juridical)

Duly accomplished Juridical Firearm
License Renewal Application Form
- Original or authenticated photocopy of old license
- Original or
authenticated photocopy of Special Bank Receipt (SBR)
- Official
Receipt of Firearms Bond (except for SP licensees)
- Document
validating a change in the data reflected in the firearms license (if any)
- Valid license to operate (for Security Agency and
Company Guard Force)
- Drug test result of owner or
manager (authenticated by the Chief, PNP Crime Laboratory Service ,
or Chief, Regional Crime Laboratory Office or their
authorized representatives)
Instructions to
Applicants

Fill-up all information required and attach your 2x2
colored picture in the space indicated.
- Attach the
required original or authenticated photocopy (duly authenticated) by the
agency/ office issuing the document.
- If there are
any changes in in any information in your firearm license (I.e. address,
qualification, etc.) indicate the changes in the appropriate spaces. Attach
a copy of the document validating the change.
- Application
with lacking requirements will note be accepted for processing.
